You have the right to receive a Good Faith Estimate explaining how much your care will cost.

what this means

Under federal law, health care providers must give patients who do not have insurance, or who are not using insurance, an estimate of the expected charges for medical items and services. Hara Center, LLC is a cash-pay practice, so this right applies to every patient we see.

what you can expect from us

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs such as laboratory testing, medications administered in the office, and supplies.
  • You can ask us for a Good Faith Estimate before you schedule, and at any time. Ask us in person, by telephone, or by email, and we will put it in writing.
  • When you schedule an item or service, we will give you a Good Faith Estimate in writing within the timeframes federal law requires, before your visit.
  • If you receive a bill that is at least $400 more than the total on your Good Faith Estimate, you may dispute the bill through the federal patient-provider dispute resolution process. You must start that process within 120 calendar days of the date on your bill.
  • Keep a copy of your Good Faith Estimate.

what a good faith estimate is and is not

A Good Faith Estimate shows the cost of items and services that are reasonably expected. It is an estimate, not a bill, and it does not include anything you may need that is not known when the estimate is made. If your care changes, we will discuss the cost with you and issue a new estimate where required.
